Before filling out a registration form, check you are in our catchment area.

Registering with the surgery
To register with the surgery you will need to complete two forms:
- Form 1: NHS Family Doctor Registration Form (GMS1 form) – you can either complete this online or download and complete at home
- Form 2: Market Cross Surgery New Patient Form – you can download this form and complete at home
- visit reception from 10am to 6pm and collect these two forms
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please ensure you complete all the sections and date both forms, otherwise we will not be able to register you.
If you would like to register for online services you will also need to bring one form of in date photo ID (eg Drivers License, Passport, Student ID, work or travel cards) when you return your completed forms.
New patients aged 40+ will need to see a nurse for a new patient medical. Patients aged 16-39 will only need a new patient medical if on regular medication.
All patients must bring proof of the medication they are taking and give to the nurse who will pass on to the GP. Patients will no longer need an appointment to see a GP to have their medication issued.
If you are on regular medication you will need to obtain one months supply from your current practice before submitting these forms.
It is important that you inform us if your details change at any time whilst you are registered at the practice.
Named GP
All patients at Market Cross Surgery have a named, accountable doctor who is responsible for coordinating their care.
Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.
