If you are travelling abroad you may need to be vaccinated. You will need to complete our Travel Health Assessment Form by downloading it or collecting one from reception. Our practice nurse will then assess your needs, which will depend on where you are travelling. One form will need to be completed by each family member travelling.
It is important to complete and return these forms at least eight weeks before you travel so that the vaccines can be ordered. Our team will then contact you to arrange your appointment to receive your vaccinations if you are elgibile.
Some travel vaccines are ordered on a private prescription which cost more than the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
